Modern medicines have contributed to longer life spans and improved quality of life. However, when not used appropriately, effectively, and safely, medications can have devastating consequences. There are several risk factors that can lead to polypharmacy, including having multiple medical conditions managed by multiple prescribers. Monitoring patients’ active medication lists and deprescribing any unnecessary medications are recommended to reduce pill burden, risks of adverse drug events, and financial hardship.
