The Hospice pharmacy strives to serve our patients with the highest quality of care and is available to you for collaboration and/or assistance to provide the most efficacious therapy, while maximizing the patient's quality of living. Your suggestions are always welcome to improve our service.

What types of patients does the Hospice Pharmacy Serve?

All patients facing the end of life, enrolled in The Hospice program residing in their private homes or in other settings, can be served by the pharmacy

What Medications are provided by the Hospice?

Following Medicare guidelines, all medications that are reasonable and necessary to palliate the symptoms of the end-stage diagnosis are paid for by The Hospice.

Does The Hospice Pharmacy Use a Drug Formulary?

Yes. A Preferred Medication List has been selected through collaboration with community and hospice physicians and clinical staff. When a non-formulary medication is ordered, the prescriber is contacted with a Formulary alternative via telephone or fax.

Monday, Tuesday, Thursday, and Friday: 9:00am –7:00pm

Wednesday: 9:00am – 6:00pm

A Pharmacist is always on call for clinical consultation and emergency orders.
